DECLARA\u00c7\u00c3O PARA CURSOS PRESENCIAL<\/p>

As solicita\u00e7\u00f5es de carteiras estudantis para estudantes matriculados em cursos presenciais nas macrorregi\u00f5es dever\u00e3o atender \u00e0s seguintes diretrizes: A declara\u00e7\u00e3o de estudante deve ser carimbada e assinada pela institui\u00e7\u00e3o de ensino e, caso seja emitida pela internet, deve conter um c\u00f3digo para confirma\u00e7\u00e3o digital.<\/p>

A declara\u00e7\u00e3o deve incluir as seguintes informa\u00e7\u00f5es obrigat\u00f3rias:
A) O tipo do curso (Presencial);
B) O CNPJ da institui\u00e7\u00e3o de ensino;
C) A cidade onde as aulas s\u00e3o ministradas.<\/p>

DECLARA\u00c7\u00c3O PARA CURSO EAD<\/p>

As solicita\u00e7\u00f5es de carteiras estudantis para estudantes matriculados em cursos EAD nas macrorregi\u00f5es dever\u00e3o atender \u00e0s seguintes diretrizes: A declara\u00e7\u00e3o de estudante deve ser carimbada e assinada pela institui\u00e7\u00e3o de ensino e, caso seja emitida pela internet, deve conter um c\u00f3digo para confirma\u00e7\u00e3o digital.<\/p>

A declara\u00e7\u00e3o deve incluir as seguintes informa\u00e7\u00f5es obrigat\u00f3rias:
A) O tipo do curso EAD;
B) O CNPJ do Polo de ensino;
C) A cidade onde as aulas s\u00e3o ministradas.
D) O endere\u00e7o Polo de Ensino deve estar obrigatoriamente cadastrado no sistema do Minist\u00e9rio da Educa\u00e7\u00e3o (Portal e-MEC).<\/p>

\u2713 DOCUMENTOS
A) C\u00f3pia do documento oficial de identifica\u00e7\u00e3o com foto e CPF do estudante;
B) C\u00f3pia do Comprovante de resid\u00eancia em nome do estudante ou de um de seus pais, com o mesmo sobrenome (conta de \u00e1gua, luz ou telefone); falta do comprovante referido na al\u00ednea anterior;
C) Declara\u00e7\u00e3o de resid\u00eancia assinada, em caso de falta do comprovante referido na l\u00ednea anterior;
D) 1 (uma) foto 3×4 recente, com fundo branco.<\/p>

PARA AS ESCOLAS MUNICIPAIS, ESTADUAIS E PARTICULARES, DEVER\u00c3O SER INFORMADOS OS DADOS DO INSTITUTO NACIONAL E PESQUISA EDUCACIONAL AN\u00cdSIO TEIXEIRA (INEP \/ MEC).<\/p>
